Medical Case Manager – Bilingual in English and Spanish

Provides all aspects of medical case management services for individuals living with HIV. Performs intakes and psychosocial assessments. Develops care plans with clients focusing on access to and continuation in medical care and treatment adherence. Identifies resources, advocates and provides guidance/referrals. Facilitates clients’ access to medical treatment, insurance coverage, substance abuse treatment, mental health care, entitlements, community based programs and other resources. Educates clients on health information including risk reduction. Monitors clients’ progress and performs required follow up. Engages Spanish speaking communities and providers in Metropolitan Detroit to develop or expand access to services. Maintains documentation in client files and computer data system according to agency standards. Data and Quality Coordinator

The Data and Quality Coordinator (DQC) will plan, design, implement, and maintain a comprehensive practice of quality management in patient care and client services at HELP and Corktown Health Center. The DQC identifies strategies to meet objectives defined by executive leadership and program/contract requirements. The DQC will work closely with clinicians, program managers, quality committee members, other assigned staff, Client Advisory Board (CAB) members and community partners when appropriate to collect and analyze data in order to address quality concerns, improve performance measures, and to improve health outcomes for patients/clients.

EIS Specialist – Bilingual in English and Spanish

Provides outreach and early intervention services to identify and link into medical care individuals who are HIV positive. Predominantly works with those who are newly diagnosed or not engaged in care and other services.

Michigan Licensed Masters Clinical Social Worker

The clinician provides general psychotherapy services (individual and some couples and group therapy) with a focus on serving adults who are HIV positive and/or identify as LGBTQ+.
