Aging Services
Aging services aims to provide a safe, culturally competent environment dedicated to the health and well-being of LGBTQIA+ adults age 60 and older.
Behavioral Health
The Behavioral Health Services (BHS) program provides mental health support to the Corktown community – available for individuals, couples, and groups, and services are customized to address the unique needs of each client we serve.
Care Management
Care Management is a collaborative process of assessing, planning, facilitating, coordinating, evaluating, and advocating for individual’s needs through frequent communication and by providing resources to clients/patients.
Financial Assistance
The Housing and Emergency Financial Assistance (EFA) programs focus on addressing financial barriers (housing and utility payments) that restrict a client’s ability to address their medical needs.
Fitness & Nutrition
This program helps patients living with HIV manage their weight and blood pressure. Goals are achieved through physical activity and nutrition courses/interventions.
Food Bank
The Food Bank program provides clients living with HIV nutritional services such as food vouchers, nutritional supplements, and Medical Nutritional Therapy with our Registered Dietitian.
Health Insurance Navigation
Our team of Certified Application Counselors (CACs) can assist any community member or patient in accessing health insurance. We will help you determine which insurance(s) you are eligible for and assist you in navigating the insurance marketplace.
Medical Care
Corktown Health provides both primary and HIV medical care to our community.
PrEP & PEP
Pre-Exposure Prophylaxis & Post Exposure Prophylaxis treatment designed to reduce or control the risk of infectious diseases.
Tobacco Cessation
The Tobacco Cessation program team provides patient-centered services focused on tobacco use reduction and to help current tobacco users quit by gaining access to medical care and resources.
