Aging services aims to provide a safe, culturally competent environment dedicated to the health and wellbeing of LGBTQIA+ adults age 60 and older.
Senior Health Clinic
Our board certified geriatric provider offers a senior health clinic every Wednesday (9:30am-12:30pm) with both in-person and telehealth services. Health screenings offered during these appointments include dementia, anxiety, depression, cancer, etc.
Insurance Navigation &
Care Coordination
Our team of care managers and insurance navigators can assist patients with Medicare insurance navigation and the coordination of additional resources (i.e. transportation, housing, support groups).

Yes. Corktown is a Michigan Medicare/ Medicaid Assistance Program (MMAP) counseling site. We have a team of certified MMAP counselors to assist clients with navigating Medicare insurance needs.
We offer multiple transportation options. We assist in helping patients navigate transportation through their medical insurance company. We also offer bus tickets and Lyft rides for eligible patients. Contact us today to learn more about our transportation options.
