POSITION SUMMARY: The Program Manager provides staff supervision and administers services within the Preventive Health Department. Responsibilities include: developing and implementing policies and procedures; supervising, training and mentoring team members; implementing, monitoring and advancing quality and performance outcomes; ensuring compliance with relevant standards of care, laws and regulations. Program managers may provide direct care/case management client services as determined by program/staff needs and to maintain familiarity with service delivery.
ESSENTIAL DUTIES & RESPONSIBILITES:
- Works with the VP of Programs & Community Impact and leadership team to promote the organization’s mission and strategic objectives within assigned areas. Develops ideas for enhancing existing programs or implementing new services within this framework.
- Engages program coordinators, associate managers and program leads as direct reports within identified areas to fulfill responsibilities and objectives. Provides mentorship and supports leadership opportunities for program coordinators, associate managers and program leads.

- Represents the agency at community meetings and venues. Develops and disseminates communication tools to reach target populations for assigned program areas.

REQUIRED EDUCATION AND EXPERIENCE:
- Master’s degree in social work required. Full licensure required. Extensive professional experience may be considered in lieu of degree requirements.
- A minimum of 5 years’ experience in social services, health care, public health or a related field and 2 years’ management/supervisory experience required.

POSITION SUMMARY:
The Vice President of Programs & Community Impact serves as a senior leader responsible for a diverse array of programs across the organization. This individual develops, implements, leads and evaluates activities in Behavioral Health, HIV Services, Prevention, Support Services and community engagement. The VP supervises program managers and implements organization-wide initiatives as part of the executive team. The VP oversees the agency’s portfolio of contracts and grants in the assigned program areas including Ryan White, Michigan Department of Health and Human Services (MDHHS), Centers for Disease Control & Prevention (CDC), AIDS United and private foundations.
ESSENTIAL DUTIES & RESPONSIBILITES:
- Supervises program managers in Behavioral Health, Preventive Health, Financial Support Services and Care Management. The scope of programs includes clinical social work, case management, physical activity & health coaching, nutrition support, housing, emergency financial assistance, food assistance, tobacco cessation, HIV counseling & testing and support for other chronic health issue initiatives. Ensures program activities are delivered effectively, organization objectives and grant requirements are met, and that staff receive appropriate direction, oversight and support.
- Coordinates timely documentation, data collection and reporting; monitors contract requirements including service objectives/work plans, performance measures, standards of care and budgets to ensure adherence. Leads preparations for site visits and audits.

REQUIRED EDUCATION AND EXPERIENCE:

POSITION SUMMARY: The Case Manager is responsible for providing accessible & culturally competent case management services to individuals living with HIV including, but not limited to, assessment of need, service planning, reassessments, treatment education, adherence counseling, linkages and referrals to public and privates services and programs, information dissemination, financial resource management, advocacy, mental health assessment, service plan follow-up, and discharge planning.
ESSENTIAL DUTIES & RESPONSIBILITES:
- Promotes the organization’s mission, vision, and strategic objectives within assigned area.
- Knowledge of HIV including risk factors, medical care needs, psychosocial challenges and community resources.
- Conducts intakes and psychosocial assessments within required timeframe, ensuring complete, accurate and timely case notes, record-keeping, and reports. Maintains thorough and complete programmatic records to include documentation of services provided and outcomes.
- Develops care plans with clients focusing on access to and continuation in medical care and treatment adherence.

HIV Prevention Specialist
POSITION SUMMARY: The HIV Prevention Specialist provides prevention education, outreach, CTR (Counseling, Testing & Referral) services, PrEP navigation & support, coordination of care and assistance with accessing insurance & benefits enrollment.
ESSENTIAL DUTIES & RESPONSIBILITIES
· Identify and link individuals interested in PrEP with medical providers.
· Coordinate care, provide insurance navigation and assist with resources and referrals for prevention clients/patients to ensure uninterrupted health care and medication access.
· Provide outreach and education in community and health care settings regarding HIV prevention including PrEP navigation, HIV testing, and agency services. May train and coordinate team members and volunteers in outreach efforts.
· Perform client intakes, review documents and enroll clients based on established policies and procedures.
· Develop and follow prevention treatment plans in conjunction with each client. Coordinate services with medical providers. Assist and refer clients to address barriers to prevention goals and PrEP uptake/adherence.
· Complete appropriate documentation and enter data for all services rendered in designated systems.
· Provide HIV education about risk reduction, testing, U=U, Treatment as Prevention and other related topics.
· Develop and follow processes for distributing risk reduction and hygiene supplies; monitor and track such supplies.

REQUIRED EDUCATION & EXPERIENCE
