POSITION SUMMARY: The HIV Prevention Specialist provides prevention education, outreach, CTR (Counseling, Testing & Referral) services, PrEP navigation & support, coordination of care and assistance with accessing insurance & benefits enrollment.

ESSENTIAL DUTIES & RESPONSIBILITIES

· Identify and link individuals interested in PrEP with medical providers.

· Coordinate care, provide insurance navigation and assist with resources and referrals for prevention clients/patients to ensure uninterrupted health care and medication access.

· Provide outreach and education in community and health care settings regarding HIV prevention including PrEP navigation, HIV testing, and agency services. May train and coordinate team members and volunteers in outreach efforts.

· Perform client intakes, review documents and enroll clients based on established policies and procedures.

· Develop and follow prevention treatment plans in conjunction with each client. Coordinate services with medical providers. Assist and refer clients to address barriers to prevention goals and PrEP uptake/adherence.

· Complete appropriate documentation and enter data for all services rendered in designated systems.

· Provide HIV education about risk reduction, testing, U=U, Treatment as Prevention and other related topics.

· Develop and follow processes for distributing risk reduction and hygiene supplies; monitor and track such supplies.

• Upholds compliance efforts. Ensures confidentiality and privacy practices in accordance with agency policies, HIPAA and relevant laws, regulations and contractual obligations.

· Other duties as assigned.

REQUIRED EDUCATION & EXPERIENCE

· Bachelor’s Degree in a health, education, or human service related field and two year’s relevant work experience required. Extensive experience may be considered in lieu of a degree.

· Excellent customer service and communication skills as well as the ability to professionally interact with individuals from diverse populations and accommodate social and cultural differences.

· Strong organizational skills and computer literacy including proficiency with Microsoft Office applications are required. · Knowledge of HIV, prevention and PrEP is strongly preferred.

· Valid Michigan driver’s license and access to insured personal vehicle is required for occasional client transportation.

· Successful completion of Counseling, Testing & Referral certification within 90 days of employment.

SUPERVISORY RESPONSIBILITIES

This position does not supervise staff.

TRAVEL

This position requires some local travel in metropolitan Detroit, limited in-state travel for meetings/trainings and may include one out of state conference/training per year.

SALARY RANGE & BENEFITS

· $17.00-$21.00/ hourly

· Benefits include paid time off, agency recognized holidays, 401(k), medical, vision, dental, disability, and life insurance after 60 days, subject to agency and insurance eligibility criteria.

SCHEDULE & WORKING CONDITIONS

This position is full time. Although work will predominantly take place during daytime and early evening hours Monday through Friday, this position does require some later evening and weekend hours for outreach. Work is generally performed within an office and/or clinic environment and includes significant in person, phone and email contact with clients, staff and the general public. This position reports to the Program Manager.
