LGBTQ+ Health and Social Services Disparities Request for Proposals

Along with multiple leading LGBTQ+ organizations across the state of Michigan, Corktown Health is pleased to announce a new funding opportunity focused on addressing health disparities within the LGBTQ+ community.

We invite community organizations
to submit proposals that aim to:
  • Identify and understand specific health disparities affecting LGBTQ+ individuals.
  • Develop innovative interventions or programs to reduce these disparities.
  • Promote inclusivity, equity, and culturally competent care.

Funding Details

  • Total available funding: $250,000
  • Award amount per organization:
    $5,000 – $49,999.
  • Grant duration: October 1, 2024 – April 30, 2025.
  • Proposal deadline: August 22, 2024 by 11:59pm
    • Additional proposals may be accepted on a rolling basis, until all funds are exhausted.
  • Awards will be announced 30 days after submission deadline.

  • Non-profit, community based LGBTQ+ mission centric organizations in Michigan.
  • Collaborative proposals are encouraged.

A proposal typically includes:

  • Proposed strategies/interventions
  • Evaluation and Metrics

Organizations review the RFP, prepare their proposals by answering questions outlined in the RFP, and submit the requested materials by the specified deadline. Proposals should address all requirements outlined in the RFP.

All proposals will be evaluated by an expert panel utilizing a standardized evaluation checklist/tool and each proposal will scored. This checklist/tool ensures all sections in the RFP have been covered, the project addresses health disparities, and promotes collaboration and innovation.

Additional proposals may be accepted on a rolling basis until all funds are exhausted.

Funds are reimbursed once the participating organization’s monthly financial status report is approved through the Michigan Department of Health and Human Services.
