Corktown Health is recruiting for an exceptional Primary Care Physician (MD/DO) to join our growing team in Detroit and Hazel Park. The Physician will provide comprehensive primary care for a diverse patient population with a focus on LGBTQ+ individuals and other underserved communities.

In addition to core primary care services, the Physician will provide or support:

  • HIV treatment and prevention services, including Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP)
  • Sexual health services including STI screening, treatment, and prevention
  • Gender-affirming care, including hormone therapy for transgender and gender diverse patients
  • Integrated care in collaboration with behavioral health, dental, and social support services

REQUIRED EDUCATION & EXPERIENCE

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
  • Board certified or board eligible in Family Medicine, Internal Medicine, or related primary care specialty
  • Active Michigan medical license (or eligibility to obtain) and current DEA registration required
  • Minimum of 2–3 years of clinical experience preferred, particularly in community health, FQHC, or underserved settings
  • Experience providing care to LGBTQ+ individuals, patients living with HIV, and individuals with complex social and behavioral health needs strongly preferred

POSITION SUMMARY: Corktown Health is recruiting for the position of Certified Medical Assistant (MA). The MA will perform administrative and clinical duties under the direction of a physician, PA or nurse practitioner. POSITION SUMMARY:

The clinician provides general psychotherapy services (individual and some couples and group therapy) with a focus on serving adults who are HIV positive and/or identify as LGBTQ+.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Assesses and evaluates individuals through interviews, case histories, observational techniques, and other methods. Selects, administers and interprets psychological tests to assess behaviors, intelligence, aptitude, abilities or interests. Analyzes data to identify symptoms and diagnose clients within the scope of the program and licensure.
  • Develops therapeutic treatment plans based on individual interests, abilities and needs of clients. Counsels clients to help them understand personal or interactive problems, define goals, and develop realistic action plans.
  • Advises clients on the potential benefits of counseling; Makes referrals to specialists or other providers/programs as appropriate.
  • Collaborates with team members in behavioral health, medical providers, consulting psychiatrists, case managers and support programs to coordinate services effectively for clients.

REQUIRED EDUCATION AND EXPERIENCE:

  • Master’s Degree in Social Work required.
  • Michigan licensure or limited licensure as a Masters Social Worker with a Clinical Specialty required.
  • Credentialed with major Medicaid plans strongly preferred.
  • Two years of experience providing therapy to adults required including experience in integrating substance abuse and medical co-morbidities with mental health issues.
  • Counseling skills including social perceptiveness, learning strategies, critical thinking, active listening/learning, deductive and inductive reasoning, sensitivity and complex problem solving.

We are looking for a compassionate, skilled, and culturally competent Dental Hygienist to join our team. The ideal candidate will have a deep understanding of, or willingness to learn about, the unique dental health challenges and concerns within the LGBTQ+ community.

ESSENTIAL DUTIES & RESPONSIBILITES:

  1. Deliver preventative dental care and treatment services, including patient education, cleanings, and routine examinations.
  2. Take and develop dental x-rays.
  3. Administer sealants and fluoride to enhance teeth protection.
  4. Chart patient treatment plans and educate patients on oral health care.
  5. Partner with and take direction form dentists to ensure preventive care is provided and that patients are referred for additional dental treatment if needed.

ESSENTIAL DUTIES & RESPONSIBILITES:

  • Promotes the organization’s mission, vision, and strategic objectives within assigned area.
  • Knowledge of HIV including risk factors, medical care needs, psychosocial challenges and community resources.
  • Conducts intakes and psychosocial assessments within required timeframe, ensuring complete, accurate and timely case notes, record-keeping, and reports. Maintains thorough and complete programmatic records to include documentation of services provided and outcomes.
  • Develops care plans with clients focusing on access to and continuation in medical care and treatment adherence.

Position Summary: The Registered Dietitian (RD) delivers medical nutrition therapy services and diabetes education (CDCES) for clients/patients at CH including LGBTQ+ and HIV positive populations. The RD designs and implements nutrition interventions, teaches clients/patients in individual and group settings, collaborates with medical providers and support staff. The RD is responsible for documenting nutrition information, creating and analyzing reports in the EHR and other systems.

Essential Duties and Responsibilities:

  • Performs clinical nutritional assessments. Evaluates individual dietary needs of clients in person taking into account their health conditions and medications; develops nutrition plans instructing and educating individuals on nutritional principles, dietary needs, food selection and preparation. Suggests diet modifications and develops meal plans and teaching materials.
  • Develops and implements policies and procedures pertinent to food & nutrition programs.
  • Teaches/mentors other staff members and may precept dietetics students and/or interns.
  • Prepares and performs food demonstrations for clients and documents goals and milestones in EHR.
  • Participates in quality management and performance improvement efforts.
  • Performs client intakes, reviews documents and assesses eligibility for programs based on established referrals, policies and procedures. May assist with distributing food vouchers and supplements for eligible low income clients.
  • Educates and supports clients affected by diabetes to understand and manage the condition. Promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes.

Required Education and Experience:

  • American Dietetic Association Registration (R.D.) required.
