Corktown Health is recruiting for an exceptional Primary Care Physician (MD/DO) to join our growing team in Detroit and Hazel Park. The Physician will provide comprehensive primary care for a diverse patient population with a focus on LGBTQ+ individuals and other underserved communities.

In addition to core primary care services, the Physician will provide or support:

• HIV treatment and prevention services, including Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP)
• Sexual health services including STI screening, treatment, and prevention
• Gender-affirming care, including hormone therapy for transgender and gender diverse patients
• Integrated care in collaboration with behavioral health, dental, and social support services

REQUIRED EDUCATION & EXPERIENCE

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
• Board certified or board eligible in Family Medicine, Internal Medicine, or related primary care specialty
• Active Michigan medical license (or eligibility to obtain) and current DEA registration required
• Minimum of 2–3 years of clinical experience preferred, particularly in community health, FQHC, or underserved settings
• Experience providing care to LGBTQ+ individuals, patients living with HIV, and individuals with complex social and behavioral health needs strongly preferred
• Experience or willingness to provide gender-affirming care and HIV prevention/treatment services
• Commitment to trauma-informed, culturally responsive care
• Must be authorized to work in the United States on a full-time basis

Salary Range & Benefits

• $200,000- $240,000 annual salary range

Medical Assistant (MA/CMA) - Part Time

POSITION SUMMARY: Corktown Health is recruiting for the position of Certified Medical Assistant (MA). The MA will perform administrative and clinical duties under the direction of a physician, PA or nurse practitioner. Administrative duties may include scheduling appointments, maintaining medical records, billing, interacting with other providers and data entry in various systems. Clinical duties may include preparing and maintaining exam rooms, taking and recording vital signs and medical histories, preparing patients for examination, drawing blood, and administering medications as directed by a clinician. The MA is expected to work as a member of a multidisciplinary team which includes behavioral health, nutrition and social work.

REQUIRED EDUCATION AND EXPERIENCE:

• Graduate of an accredited Medical Assisting Program and evidence of current MA certification
• Minimum 4 years of clinical experience (preferably providing care to LGBTQ+ persons and/or people living with HIV)
• Experience using an electronic medical record systems to schedule patient visits and document patient information preferably AthenaHealth
• Vaccine management, prior authorization and insurance portal experience required.
• Excellent phlebotomy skills required.
• Busy ambulatory experience required.
• Able to speak, understand, read and write fluently in Spanish preferred.

POSITION SUMMARY:

The clinician provides general psychotherapy services (individual and some couples and group therapy) with a focus on serving adults who are HIV positive and/or identify as LGBTQ+.

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Assesses and evaluates individuals through interviews, case histories, observational techniques, and other methods. Selects, administers and interprets psychological tests to assess behaviors, intelligence, aptitude, abilities or interests. Analyzes data to identify symptoms and diagnose clients within the scope of the program and licensure.
• Develops therapeutic treatment plans based on individual interests, abilities and needs of clients. Counsels clients to help them understand personal or interactive problems, define goals, and develop realistic action plans.
• Advises clients on the potential benefits of counseling; Makes referrals to specialists or other providers/programs as appropriate.
• Collaborates with team members in behavioral health, medical providers, consulting psychiatrists, case managers and support programs to coordinate services effectively for clients.
• Maintains documentation in clients' electronic health records and other systems according to professional practice, agency standards and billing requirements. Prepares insurance related paperwork as required for reimbursement.
• Adheres to relevant standards of care, Michigan Public Health Code, Michigan Mental Health Code and relevant professional standards.
• Other duties as assigned.

REQUIRED EDUCATION AND EXPERIENCE:

• Master’s Degree in Social Work required.
• Michigan licensure or limited licensure as a Masters Social Worker with a Clinical Specialty required.
• Credentialed with major Medicaid plans strongly preferred.
• Two years of experience providing therapy to adults required including experience in integrating substance abuse and medical co-morbidities with mental health issues.
• Counseling skills including social perceptiveness, learning strategies, critical thinking, active listening/learning, deductive and inductive reasoning, sensitivity and complex problem solving.
• Experience working with LGBTQ+, HIV positive, homeless and/or chronically ill populations strongly preferred.
• Excellent communication and customer service skills as well as the ability to professionally interact with individuals from diverse populations and accommodate social and cultural differences.
• Ability to work independently, manage schedules and billing requirements with minimal supervision.
• Computer skills including proficiency with Microsoft Office applications and aptitude for learning and using electronic medical records.

POSITION SUMMARY:

We are looking for a compassionate, skilled, and culturally competent Dental Hygienist to join our team. The ideal candidate will have a deep understanding of, or willingness to learn about, the unique dental health challenges and concerns within the LGBTQ+ community.

ESSENTIAL DUTIES & RESPONSIBILITES:

1. Deliver preventative dental care and treatment services, including patient education, cleanings, and routine examinations.
2. Take and develop dental x-rays.
3. Administer sealants and fluoride to enhance teeth protection.
4. Chart patient treatment plans and educate patients on oral health care.
5. Partner with and take direction form dentists to ensure preventive care is provided and that patients are referred for additional dental treatment if needed.
6. Document in the designated electronic dental data system.
7. Advocate for patients, understanding that many may have had prior negative healthcare experiences or concerns specific to their identity.
8. Adhere to policies, procedures, workflows, sterilization processes, etc.
9. Keep abreast of the latest research and developments regarding LGBTQ+ dental health.
10. Attend community outreach events to educate and promote dental health within the LGBTQ+ community.
11. Adhere to local, state, and federal regulations and maintain current licensure and continuing education requirements.
12. Provide care that is respectful of, and responsive to, individual patient preferences, needs, and values, ensuring that patient values guide all clinical decisions.

REQUIRED EDUCATION AND EXPERIENCE:

1. Valid Dental Hygienist license in Michigan.
2. Degree from an accredited dental hygienist program.
3. Bachelor’s degree required.
4. Minimum of 3 years’ experience as a dental hygienist.
5. Current BLS certification.
6. Current certification to administer and monitor nitrous oxide and local anesthesia.
7. Knowledge of dental software.
8. Experience working with diverse populations, preferably within the LGBTQ+ community.
9. Sensitivity, understanding, and a commitment to cultural competence and inclusivity.
10. Excellent communication and interpersonal skills.

POSITION SUMMARY: The Case Manager is responsible for providing accessible & culturally competent case management services to individuals living with HIV including, but not limited to, assessment of need, service planning, reassessments, treatment education, adherence counseling, linkages and referrals to public and privates services and programs, information dissemination, financial resource management, advocacy, mental health assessment, service plan follow-up, and discharge planning.

ESSENTIAL DUTIES & RESPONSIBILITES:

• Promotes the organization’s mission, vision, and strategic objectives within assigned area.
• Knowledge of HIV including risk factors, medical care needs, psychosocial challenges and community resources.
• Conducts intakes and psychosocial assessments within required timeframe, ensuring complete, accurate and timely case notes, record-keeping, and reports. Maintains thorough and complete programmatic records to include documentation of services provided and outcomes.
• Develops care plans with clients focusing on access to and continuation in medical care and treatment adherence.
• Identifies resources, advocates and provides guidance/referrals. Facilitates clients’ access to medical treatment, insurance coverage, substance abuse treatment, mental health care, entitlements, community based programs and other resources.
• Educates clients on health information including risk reduction.
• Monitors clients’ progress and performs required follow up.
• Responds to walk-in clients and phone calls in a timely manner.
• Engages communities and providers in Metropolitan Detroit to develop or expand access to services.
• Maintains documentation in client files and computer data system according to agency standards.
• Represents the agency at community meetings and venues. Participates in outreach activities to promote services and education in the community.
• Collaborates internally and externally to maintain effective relationships.

REQUIRED EDUCATION AND EXPERIENCE:

• Master’s Degree in social work or related field preferred. Bachelor's Degree required. Case Management certification required within one year of hire.

• Familiarity with and ability to navigate and execute software programs (i.e. Microsoft Office, Database, Electronic Health Records).

  • MI Bridges Community Partner and Navigation Referral training within first 60 days

Position Summary: The Registered Dietitian (RD) delivers medical nutrition therapy services and diabetes education (CDCES) for clients/patients at CH including LGBTQ+ and HIV positive populations. The RD designs and implements nutrition interventions, teaches clients/patients in individual and group settings, collaborates with medical providers and support staff. The RD is responsible for documenting nutrition information, creating and analyzing reports in the EHR and other systems.

Essential Duties and Responsibilities:

• Performs clinical nutritional assessments. Evaluates individual dietary needs of clients in person taking into account their health conditions and medications; develops nutrition plans instructing and educating individuals on nutritional principles, dietary needs, food selection and preparation. Suggests diet modifications and develops meal plans and teaching materials.
• Develops and implements policies and procedures pertinent to food & nutrition programs.
• Teaches/mentors other staff members and may precept dietetics students and/or interns.

• Prepares and performs food demonstrations for clients and documents goals and milestones in EHR.
• Participates in quality management and performance improvement efforts.
• Performs client intakes, reviews documents and assesses eligibility for programs based on established referrals, policies and procedures. May assist with distributing food vouchers and supplements for eligible low income clients.
• Educates and supports clients affected by diabetes to understand and manage the condition. Promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes.
• Upholds compliance with relevant standards of care. Ensures confidentiality and privacy practices in accordance with agency policies, HIPAA and relevant laws, regulations and contractual obligations.
• Documents minimum of 2 encounters per client per year in EHR and appropriately closes encounters with medical billing.
• Other individual and group duties as assigned.

Required Education and Experience:

• American Dietetic Association Registration (R.D.) required.
• Master’s degree required with Diabetes Care and Education Specialist (CDCES) Certification preferred.
• Minimum of five years’ work experience in clinical dietetics strongly preferred.
• Experience with HIV related nutrition and EHR documentation/billing preferred.

Required Knowledge, Skills and Abilities:

• Must have great clinical, strategic and organizational skills and be able to multi-task.
• Excellent leadership and customer service skills as well as the ability to professionally interact with individuals from diverse populations and accommodate social and cultural differences.
• Advanced computer skills including proficiency with Microsoft Office, Outlook, Excel, EHR and Reporting applications.
• The ability to meet in person with all clients, stand for long periods of time and lift a minimum of 30 pounds for distribution of nutritional supplements.

Travel:
This position may require limited in-state travel.
